Go here to process warranty services for customers, return DOA/Cracked Box Inventory, etc.

Metro Merchandising Portal

Go here to view the latest requirements around how your store should look and feel. Commonly known as MVP.

Phone Inventory Request

We try to keep as much in stock as possible. But if there's an item available that you aren't seeing in store and want us to pick it up; Fill out this form.

New Hire Onboarding Form

Use this form (Managers) when you're hiring a new member to our team. Once you fill this out, our HR team will begin onboarding your candidate.

HelpDesk Support (IT)

Go here to request help on any tech related help you need. Spiked Wireless does NOT control your QPay, or Thin Client devices.

Trainee Profile Unlock

Go here to unlock profiles that are in "Trainee" mode in RTPOS. You will also use this form to unlock your profile if you've gone past due.

Employee Termination

Go here to submit employee termination paperwork (Managers). This will notify the HR team to conduct the proper exit process.

PNW Sick Time Form

For WA and OR staff members only. Request usage of WA and OR state sick time that has been accrued.

Payroll Update Form

For Managers: Submit any payroll changes that are approved by the VP of Sales and Director of Operations here with this form.

Employee Promotion Form

For Managers: Submit any promotions through this form. These must be approved with the VP of Sales and Director of Operations.

Uniform Ordering

Submit any uniform and nametag orders here on this form. You are allotted 3 shirts and a nametag when you start.

Supply Order Form

Running out of some items in your store? Submit for supplies to be purchased here with this link. Be as detailed as possible what you're requesting.

Equipment/Fixture Repairs

If an item has broken or been damaged in your store, please fill out this form here, and include details around what it is and what is wrong.
